State Street Family Practice is the Practice with Heart located in the heart of historic downtown Ephrata.
The practice is conveniently located at 11 S. State St., offers off street parking, and is handicapped accessible. The hours are Monday through Friday with occasional Saturday mornings available, plus Monday and Wednesday evening hours available to better meet the needs of our patients.

A. Peter Calusic, DO
Dr. Calusic graduated Magna Cum Laude from University of Notre Dame, and is a graduate of the College of Osteopathic Medicine and Surgery Des Moines, Iowa. He completed Family Practice Residency at Community Hospital of Lancaster, and is Board Certified in Family Practice by the ACOFP, He is past President of the Family Practice Department of the Ephrata Community Hospital, and past Chairman of the Pharmacy and Therapeutics Committee as well. He has a special interest in allergy and immunology. In addition, he is skilled with osteopathic manipulation for spinal alignment and performs minor surgeries such as removal of skin lesions. He is a highly experienced physician who cares for all ages from birth to the elderly, with special attention to hypertension, diabetes, cardiovascular disease, and cancer screening. He is a family man and a great sports fan.
James Michael Elia , MD
Dr. Elia received his medical degree from the State University of New York, Health Science Center at Syracuse College of Medicine. He completed his residency in Family Practice at Pennsylvania State University, The Good Samaritan Hospital in Lebanon, PA. Dr. Elia is Board Certified in Family Practice. He has special interests in pediatrics, adolescents, diet counseling and smoking cessation counseling. He is also skilled in minor office procedures, neonatal and nursing home care. Dr. Elia resides in Lancaster with his four children.
